In order to be able to identify you, it is required that you provide us with your full name and one valid email address, given that we have an email address on file for most individuals in our database. We recommend submitting both your most recent business email address and your personal email address to increase the chances that we can locate your record in our systems. This also allows us to provide you with a confirmation that your request is or has been processed, or to contact you if we have questions about your request and/or to verify your identity if we are uncertain that data we hold pertains to you.
